Participant Liability Waiver & Release — The Swarm Soccer LLC

This Liability Waiver and Release ("Agreement") is entered into by the participant (or parent/legal guardian if the participant is under 18 years of age) and The Swarm Soccer LLC, a Tennessee limited liability company ("The Swarm Soccer").

1. Assumption of Risk. I understand and acknowledge that participation in soccer training sessions, drills, small-sided games, and related athletic activities involves inherent risks of physical injury, including but not limited to sprains, strains, fractures, concussions, and other injuries. I voluntarily assume all such risks on behalf of myself or my minor child.

2. Release of Liability. In consideration of being permitted to participate in training sessions organized by The Swarm Soccer LLC, I hereby release, waive, discharge, and hold harmless The Swarm Soccer LLC, its coaches, staff, agents, volunteers, and affiliated organizations from any and all claims, damages, losses, or liability arising out of or related to participation in any training session, whether caused by negligence or otherwise.

3. Medical Authorization. I authorize The Swarm Soccer LLC and its staff to seek emergency medical treatment for the participant if necessary, and I accept full financial responsibility for any such medical treatment.

4. Photo & Media Release. I grant The Swarm Soccer LLC permission to use photographs, video, or other media taken during training sessions for promotional or educational purposes, without compensation.

5. Health Acknowledgment. I confirm that the participant is in good physical health and has no known medical conditions that would prevent safe participation in athletic training. I agree to inform Coach Reed of any injuries, medical conditions, or physical limitations prior to each session.

6. Conduct. I agree that the participant will behave in a respectful and sportsmanlike manner. The Swarm Soccer LLC reserves the right to remove any participant from a session or program for conduct deemed inappropriate or unsafe.

7. Cancellation & Refund Policy. I understand and agree to the following: (a) Cancellations made more than 24 hours before a session are eligible for a full refund. (b) Cancellations made within 24 hours of a session start time are non-refundable. (c) No-shows on the day of a session are non-refundable under any circumstance, as Coach Reed prepares staffing and resources based on confirmed attendance. (d) If The Swarm Soccer LLC cancels a session for any reason — including weather, safety conditions, or scheduling — the participant will receive either a full refund or a credit toward a future session at their discretion.

8. Governing Law. This Agreement shall be governed by the laws of the State of Tennessee. If any provision is found to be unenforceable, the remaining provisions shall remain in full force and effect.
